K-12 Teacher's Guide to the Library: School Visit Planning Form

The UW Libraries look forward to welcoming your K-12 group to campus! 

Please review the information below about visiting and complete the visit request form if you would like to bring your class to visit.

Guidelines

Guidelines

  • Visits must be scheduled at least three weeks in advance.
  • Visits are not possible at some times during the year – for example – the week or two prior to UW final exams.
  • Visits can include a brief tour and an overview of library resources, pending the availability of staff and rooms.
  • No printing available on-site (visitors can save to USB, scan, email, or photograph resources)
  • If visitors want to check out physical items from the UW Libraries, have a parent or guardian complete a Parental Permission form and make a copy of their government issued photo ID.  Then mail or fax these 2 items for each student to us. This will enable us to have the library cards ready at the circulation desk at the time of your arrival. Contact Coe Library Help Desk 307-766-3190, libcirc@uwyo.edu, or fax 307-766-2510
  • Arrange to have adequate adult supervision for your visit. At least one adult per ten students is recommended.

Permission Forms

To allow students to check out books from UW Libraries: 

  • A parent or guardian must complete a Parental Permission form and make a copy of their government issued photo ID
  • Mail or fax these 2 items for each student to us.
  • This will enable us to have the library cards ready at the circulation desk at the time of your arrival.

If you do not plan to allow students to check out books, we do not require a specific permission form. We do request that you collect your own permission forms for the field trip and bring at least 1 chaperone per 10 students.
